The Back-UP first contact web app provides clinicians with a technology (translated in over 10 EU languages) to help with the following tasks:• Risk-stratification items and individualised risk prediction visualisations within a shared decision-making summary dashboard for clinicians • A treatment selection tool of risk-matched evidence-based first contact options• A library of trusted patient self-management resources (in multiple languages)• Clinical questions to tailor the care plan content (& sharing method e.g. email) • A patient held personalised care plan containing their prediction visualisations, agreed treatment plan, self-management resources, and what to expect next• Peer-to-peer comparison tables of a clinician user’s clinical decision-making choices within an interactive “My statistics” section.
